This is a cute little 5 pocket holder I made from a blank template. The kit can be purchased at Pine Cone Press here. I made little cards to go inside each pocket with a tab on the top. The kit came with the white cardstock to make the folder and the pockets. It is very simple to make and decorate and was lots of fun too! Hope you like it!
